Could hormone replacement therapy boost your immune system? – Open Access Government

How good is your immune system? If you are menopausal, or postmenopausal, you may not be aware that both ageing, and lack of estrogen, affect your immune response. In the face of the COVID-19 pandemic this is especially topical.

The immune system is a complex network of specialised cells and cell signalling mechanisms. The function of the immune system is to protect your body from a harmful attack. This could be from a single bacterium, a virus, or one for your own cells which is damaged, or dead and requires clearance.

Without a healthy immune system, the human body cannot survive. Your immune system can become weakened for a variety of reasons.

The strength of your immune response naturally deteriorates as you get older often referred to as immune senescence. This is why older people seem to have more infections and are more at risk of dying from them, than younger people.

Other medical conditions can cause your immune system to become weak. For example, obesity, drinking excess alcohol or suffering from malnutrition. HIV (the human immunodeficiency virus) also damages the immune system.

Medical conditions such as Type-1 diabetes, Rheumatoid Arthritis, and coeliac disease, are autoimmune conditions, in which your bodys immune system, is mistakenly attacking itself.

There is also a difference between the male and female immune system. The female immune system seems generally stronger and reacts more vigorously to infection that is until menopause.

Estrogen levels start to decline many years before your final menstrual period. This is because your ovaries are gradually becoming less responsive, and eventually, they completely fail.

After menopause, your estrogen levels fall dramatically. They remain around 90% lower than they were in the premenopausal period. Low estrogen levels seem to be associated with a weak immune system. For example, in postmenopausal women, the following observations have been reported in medical studies

Chronic inflammation occurs when acute inflammation has taken place but failed to switch off.

Acute inflammation occurs when the body recognises a foreign substance or a damaged cell. The cell signalling process leads to a rise in the level of chemical messengers called cytokines. These put out a call for help, summoning much-needed immune cells to kill the organism, and clear away the dead cell.

However, with ageing, this acute process becomes chronic. Persistent high levels of inflammatory cytokines, instead of being protective now cause long term cell damage. This is all part of the ageing process.

Chronic inflammation is a major underlying cause of medical conditions such as atherosclerosis, obesity, diabetes and Alzheimers Disease.

Some medical evidence is now available that this seems to be the case. Below are 2 examples from the literature

In a 2004 study in the Journal of Reproductive Immunology, the authors studied a group of 17 women undergoing a hysterectomy and removal of both ovaries. They took blood samples before surgery, after 30 days, then 30 days after starting estrogen-only hormone replacement using HRT patches.

The results showed that immediately after surgery, there was a significant decrease in the ratio of CD to CD8 cells, and the levels of cytokines, serum IL-4 and INF-gamma.

After completing 30 days of estrogen replacement, the CD4 to CD8 ratio and levels of INF-alpha had significantly increased. The IL- 4 levels, however, remained unchanged.

The authors concluded that estrogen has an important role in the female immune response.

In a 2016 review in the journal Climacteric, the authors reviewed all the research studies on this topic, published 1995-2015. There were 209 studies which met the study criteria for inclusion.

The authors concluded that HRT, taken by women at menopause, can alter the immune response as a result of specific changes to various chemical mediators.

Below are some specific findings

HRT has been shown to reduce the levels of these three cytokines.

HRT has been shown to reverse these changes.

The following study reported that taking HRT lead to an improved vaccination response to a flu jab.

In a 2016 study in the journal PLOS One, the authors studied the response to a standard influenza vaccine, in 3 groups of women.

Group 1 consisted of premenopausal women having regular periods and not using any additional hormones or hormonal contraception. Group 2 were postmenopausal women of at least 2 years duration and who had never taken HRT. Group 3 were postmenopausal women who had had a hysterectomy and were taking estrogen-only HRT.

The study participants were given the influenza vaccine and had blood samples taken on day 1. They then had further samples taken after one week, and again after one month.

The results showed that although numbers of CD4 cells remained lower in the HRT group, other parameters of immune function were improved, such as improved numbers of immature B lymphocytes, reduced T cell cytokine production and a small reduction in levels of IL-6.

The premenopausal women had the best antibody response compared to either of the postmenopausal groups. However, there was a direct correlation noted between blood levels of 17estradiol in women on HRT, and levels of vaccine-specific, immunoglobulin IgG. IgG is the most common type of antibody found in the blood and has a major role in the recognition of foreign antigens and the cellular defence mechanism.

In the discussion, the authors commented that the increase in immature B lymphocytes post-vaccination is a very positive finding, as these young B cells make up an important reservoir for any infection that the body meets for the first time.

The association between 17estradiol levels and the increase in vaccine-specific IgG was only seen on day-7 post-vaccination. Similar findings have been seen in mice. The authors suggested that additional estrogen may be beneficial for a good, post-vaccination antibody response, in postmenopausal women.

There are so many issues to consider when you reach menopause, it can be hard to know what to believe and what to do for the best.

You can do a lot to strengthen your immune system by eating healthily, taking regular exercise, losing weight and getting enough sleep.

Many women are unaware that ageing, and estrogen deficiency at and after menopause, are associated with a weakening of their immune system. Also, that estrogen replacement in HRT can reverse some of these effects.

At the present time, in view of the outbreak of COVID-19, it may be comforting for women on HRT to know this is likely to be a beneficial effect. For anyone considering HRT, this information may perhaps, help you to make a decision.

The most challenging result in our study is the emerging of socioeconomic inequalities in age at first delivery, number of pregnancies, number of alive newborns, and diagnosis of fertility problems in women born after 1950. Breastfeeding, suffering abortions or dead newborns, consumption of oral contraceptives or hormonal replacement therapy were associated with socioeconomic level in women born before 1950 but not in women born after that year.

Stratifying our analysis according to being born before or after 1950 was not arbitrary. It is noteworthy that women born in that year reached their sexual maturity around 1970, which would be considered some kind of social milestone for Spanish women. Spanish women incorporated to labor market after 1970, the labor activity rate in 1976 being 55.1% for women aged 2024 (i.e., born after 1950) and only 29.7% for women aged 2559 (i.e., born before 1950) [18]. Secondly, sexual behavior began to change in most western countries in the 60s, involving usage of contraceptives, family planning and women taking more control of their sexual / reproductive lives. The Spanish society, however, had their sexual habits ruled by the dictatorship that ended in the 70s, including for instance late initiation of sexual relationships, one partner only, or inclusion of adultery in the penal laws until 1976 [19]. Therefore, sexual freedom and generalized accessibility to contraceptives was reached by Spanish women from 1976 on, with some delay respect to women in other western countries. Thirdly, a deep economic crisis affecting Western countries (the so-called first oil shock) began in 1973. This eventually led to high unemployment rates and to deep falls in birth rates, which were more intense in Spain: the average number of children per women fell from 2.90 in 1970 to 2.22 in 1980 and 1.36 in 1990 (for comparison, figures in the UK were 2.43 in 1970 and 1.83 in 1990). In the same 20-year period, the birth rate fell in Spain from 19.5 in 1970 to 15.3 in 1980 and 10.3 newborns per 1000 inhabitants in 1990 (in the UK: 16.2 in 1970 and 13.9 in 1990) [20]. Fourthly, the public health service increased its coverage from 1970 to 1980, reaching universal coverage in 1988 [21]. Finally, the continued social trend toward reduced family size instead of that in the past, in rural areas above all, where children were needed and deemed to be suitable for farming or helping at home [22].

That social context could easily explain changes when the socioeconomic inequalities decreased in women born after 1950. For instance, hormonal contraceptives were marketed in Spain from 1964 on, but their indications initially included keeping the ovary in rest, controlling menstrual cycle, and treating dysmenorrhea and acne, while their contraceptive effect was considered as an undesirable side effect [23]. For years, they were more accessible for highly educated women living in urban areas [24]. When the public health service included contraception in its portfolio, women could access it without socioeconomic differences, leading to the results we have described for women born after 1950. Hormonal replacement therapy appeared much later than hormonal contraceptives, but its usage was amplified via private practicing doctors. This eventually resulted in higher consumption for women in higher socioeconomic levels; in 2001, when most women born after 1950 had not reached menopause, the Womens Health Initiative study found an association between hormonal replacement therapy and several cancers [25] and other chronic diseases [26], leading to a dramatic decrease in hormone replacement therapy in Spain [27].

The emergence of new socioeconomic disparities in age at first delivery and number of pregnancies in women born after 1950, however, is challenging. These inequalities appeared or were intensified in an era of universal coverage of the public health service, with free access to contraceptive methods and widely available information about them. In this regard, it is noteworthy that universal health coverage does not imply -by itself- equity in health assistance. For instance, the OECD have noticed that despite the fact that most OECD countries have achieved universal health coverage, people from the most socially disadvantaged groups tend to have worse access to health services. Possible reasons include lack of awareness of health services, poorer quality of care and co-payments for care [28,29,30]. In this regard, Spanish women aged 2544 (most of their fertile age) declared having unmet needs for health care in higher percentages if their income are in the lower quintile, although the gradient associated with income level was rather mild (5.84, 3.13, 4.15, 3.52 and 3.11% for Q1 to Q5 in 2001, [31]. Admittedly, these data on unmet needs in 2001 are too late for explaining our results, but National Health Surveys carried out before 2001 did not include any question on unmet needs. Apart from this explanation, we can only speculate on whether cultural issues associated with lower both socioeconomic and educational levels or differential access to work market could have prevented women in such levels to decrease their fertility as women in higher levels did. For instance, it could be possible that women in higher socioeconomic level incorporated earlier to the work market and, thus, delayed their decisions on having their first child. It is noteworthy, in this regard, that socioeconomic-level associated inequalities were mainly in drugs usage (hormonal contraceptives and hormone replacement therapy) in women born before 1950, which could be associated with inequities in accessing health care. The main inequalities in women belonging to later generations, however, seem to be associated with their own decisions (number and age of pregnancies), not with access to medical care. Along the same lines, a recent review carried out in 2019 showed how social determinants play an important role in the stage of breast cancer in diagnosis and survival [32].

Selection of population controls is a main strength of this study. Women aged 2085years were enrolled in 12 Spanish provinces after being selected from general practitioners roasters; they can provide a representative sample of the Spanish women, given the almost universal coverage of the national health system in Spain.

Some limitations of the study should also be noted. Firstly, reproductive variables were self-reported, which could lead to recall bias; however, as women were not aware of the hypotheses of this study, we would expect that recall bias -if exists- could be non-differential. Secondly, one of the SE indicators we have use -the Urban Vulnerability Index- is ecological by nature, which makes it possible the occurrence of ecological bias. In this regard, aggregate deprivation indexes have been found good proxies of individual income but less efficient to measure education or occupational category [33].

For women with serious cases of sleep apnoea, there are masks, called CPAP (Continuous Positive Airway Pressure), which blow air into the throat to keep breathing easier.

You can replace sugar with xylitol, which is naturally found in fruits and berries and has a low glycaemic index (a measure of how fast blood sugar levels rise after eating).

Davina McCall has revealed shes still taking hormone replacement therapy (HRT) eight years after starting the perimenopause.

The TV presenter, 52, started HRT when she beganexperiencing symptoms like severe night sweats.

Speaking to Sunday Mirrors Notebook magazine, she said: Literallythe first time I put the little magic sticker on I slept through the night.

I hadnt slept through the night for years, Id been lyingon a towel because the sheets were so wet. I felt back to me again. I went fromfeeling desperate to really amazing overnight.

Davina admitted she would stay on the pills until shes 80and even asked the doctor if she could.

He said, Lets just take it year by year, she explained. But at the moment I still feel amazing.

HRT is a treatment to relieve symptoms of the menopause. Itreplaces hormones that are at a lower level as you approach the menopause.

Symptoms of menopause include hot flushes, night sweats,mood swings, vaginal dryness, and reduced sex drive.

The star also recently opened up about her terrifying drug overdose around 30 years ago.

She explained that drugs took her to a very, very, verydark place, and recalled the terrifying night she overdosed.

It was very frightening as it was a very gradual overdose.Id had a cocktail of drugs, because I didnt want to feel normal, sherecalled.

I was constantly trying to escape myself I was like a piece of fruit rotting from the inside.

Speaking at The Sun on Sundays International Womens Day event, she added: So then I thought, Well, Ill just take a valium, and the next thing I knew I woke up in hospital. At my lowest point, when I was filled with self-hatred, it was literally a case of get clean, or die.

Davina went on to say that she began to attend Narcotics Anonymous meetings, and had quit drugs within a year, before mentioning that she still sometimes goes if she feels she needs it.

The extraordinary life and times of Genesis P-Orridge – Louder

In January 1971, 20-year-old Neil Andrew Megson changed his name by deed poll. It was one of the only times we really upset my mum, he recalls. We were hitching down to London and stopped off at my parents place: Erm, theres something we need to tell you. Were not Neil Megson anymore, were now Genesis P-Orridge. And she just started crying. She saw it as a rejection. And it was, but not of her. It was a rejection of the traditional way people labelled and limited their culture.

This rejection of accepted societal norms has been a central theme in the work of Genesis P-Orridge. Its there in the industrial anti-rock of Throbbing Gristle, the band he founded in 1975, and in his subsequent tenure as leader of post-punk deviants, Psychic TV. Both groups thrived on chaos and confrontation, their music a constantly evolving mix of brutal noise, avant-pop and strange electronica. Their lyrical preoccupations too illicit sex, murder, ritual, fascism, the occult were designed to shock, to provoke extreme reactions.

Above all, the idea that identity is fluid rather than fixed is reflected in Genesis P-Orridge himself. If prog is often defined as the impulse to break down barriers and taboos, to explore new opportunities, then he is its living embodiment. Along with second wife Jacqueline Breyer, the 90s saw him begin a series of body modification experiments that involved cross-dressing, breast implants and hormone replacement therapy in order to merge as one pandrogynous entity called Breyer P-Orridge. Another detail was the adoption of neutral gender pronouns (each of them now being addressed as s/he), which also explains why Orridge now refers to himself using the plural we as opposed to the singular I.

Confused? For Genesis, its actually very simple: Theres a phrase that sums it up: Viva la Evolution. I have a patch on the back of my denim jacket that Lady Jaye [aka Breyer] gave me, that just says: Change. Its important for people to understand that they really can be anyone they want to be. Jaye used to say: Why wake up in the morning and be who you wereyesterday?

Lady Jaye died in 2007 from a heart attack, probably related to stomach cancer. Genesis has now documented h/er life, and the couples relationship, in a hefty new book. Genesis Breyer P-Orridge, gleaned from reams of personal archives, is a tome that combines over 350 photographs (some of which, be warned, are very intimate), essays, artwork, commentary and memorabilia.

Its been an extraordinary life. Born in post-war Manchester, h/er musical tastes were initially inspired by h/er father, a Dunkirk veteran with a predilection for jazz. In my early teens, he started taking me to gigs at the Free Trade Hall. We saw Duke Ellington, Count Basie, Buddy Rich, all kinds of amazing jazz musicians. A lot of it was based on improvisation, and that always made sense to me. It was like: Why should it always be the same?

In the 60s, at Solihull School in the Midlands, Orridge discovered the writings of the Beats. H/er imagination was fired by Kerouac, Ginsberg and especially the random cut-up techniques of William Burroughs and Brion Gysin. Then came Dada, Surrealism and Andy Warhol. These peoples lives were as fascinating as what they made, often even more so. It fitted in with how we felt, that it was more like a calling to be an artist of any type. It was like being a priest or doctor. Its a mystical, spiritual activity. Life and art can never be separated.

Then came a fascination with Captain Beefheart, Frank Zappa and Syd Barretts Pink Floyd. After dropping out of Hull University in 1969, s/he joined a semi-commune of freaks holed up in an old warehouse in the city. The university would call whenever they needed anything strange to accompany the many visiting bands who played there.

Orridge in particular remembers Pink Floyd breezing in to tour Ummagumma: We were co-opted as an additional section of the light show. We were so excited at being asked that we ate these hash cookies on the way there, so everyone was stoned and tripping on top. We had these glass slides with liquid in, then also an epidiascope, onto which we put live maggots. So you had this amazing psychedelic light show, with six-foot long maggots crawling across. A lot ofpeople forget Im absolutely a child of the 60s. Throbbing Gristle was hardpsychedelics.

Later that year, inspired by the subversive prankery of Dada and the Situationists, Genesis co-founded the performance art ensemble COUM Transmissions. Alongside Peter Sleazy Christopherson, Chris Carter and Christine Newby (aka Cosey Fanni Tutti), it was the same line-up that would eventually morph into Throbbing Gristle.

At the beginning COUM was basically absurd theatre, s/he explains. People hated us. None of us could play and we just saw it as a big laugh. But as it got more serious, we started homing in on sexual taboos, transgression and who decides whats okay in certain societies. We were seeing how far we could push the envelope of insanity. We had an amplified shop dummy that wed bang with sticks to make rhythms. And one of the singers was on a surfboard on a bucket of water. Cosey was wandering around with a starting pistol, dressed as a schoolgirl. And Sleazy started collaging background tapes of sound.

The transition to Throbbing Gristle coincided with their controversial show Prostitution, held at Londons ICA in 1976. The happening included pornographic images, soiled nappies and an art deco clock stuffed with used tampons under the title: That Time Of The Month. There were questions asked in Parliament. Tory MP Nicholas Fairbairn called the group wreckers of civilisation, a quote gleefully appropriated by TheDaily Mail.

The bands early gigs werent received much better. The first review said: Even an ape with severed arms can play bass better than Genesis. And someone else wrote about my violin: Ive never seen such utter degradation of a beautiful instrument before. We felt encouraged!

With Genesis on vocals, Throbbing Gristle were an uncompromising bunch. Debut LP Second Annual Report was a dense mass of industrial noise that included such queasy listening as Maggot Death and After Cease To Exist, the latter featuring the taped confession of a real-life killer.

For all the disturbing aspects of their work however, it wasnt without black humour and self-mockery. 1978 single United was a favourite in the punk clubs, whereupon TG promptly sped it up as a remixed version that lasted just 16 seconds, while Death Threats, which appeared on that years DOA: The Third And Final Report, was a collage of abusive messages left on their answerphone.

By 1981 though, Orridge had become restless. We were disillusioned and in a rut. TG was becoming commercially viable, but at the same time it was becoming stale. So being an idealist, we just thought it was time to stop.

S/he had been dreaming up a new venture for some time. The sleeve for TGs final single, Discipline, had included the description: Marching Music for Psychick Youth. Plus some of their flyers had carried the words: Psychick Youth Rally!

We were deliberately trying to sow the seeds of Psychic TV before the end of TG, s/he admits. We wanted to do something that was much more positive. Its alright to be negative, like TG could be, and say everything sucks. But after a while you have to say: Well what are you gonna do about it? So the idea was for Psychic TV to not just be a band, but also a network where the people who would normally just be fans are in more direct touch. And it grew into this alternative way of doing culture.

Co-founded by Orridge and Alternative TVs Alex Fergusson, with Peter Christopherson joining soon after, Psychic TV were far more free-ranging than TG. There was still a fair spread of juddering mechanical noise, but also a raw minimalism that allowed room for electronic pop, day-glo psychedelia and the liberal use of samples. By 1988 they were even at the forefront of the rave explosion with the single Tune In (Turn On The Acid House) and LP, Jack The Tab.

There were more diverse musical wanderings up until the late 90s, when Orridge decided to call it quits and concentrate on spoken-word projects. He revived Psychic TV for 2007s Hell Is Invisible Heaven Is Her/e and follow-up Mr Alien Brain vs. The Skinwalkers.

The bands present compass points to all things prog. In the early 70s, Orridge devoured Amon Duul, Can, Jan Dukes de Grey and Hawkwind. Prog had improvisation, which was interesting. And weve rediscovered it now with Psychic TV. Were doing a series of 12-inch singles. The first one was Funkadelics Maggot Brain, but we added vocals. Then we did Mother Sky by Can, Silver Machine and Hurry On Sundown by Hawkwind.

S/he finally got to play with Hawkwinds Nik Turner in San Francisco in 1996. Genesis is a very revolutionary character, offers Turner. Psychic TV and Throbbing Gristle were vehicles for his off-the-wall expressionism. Those who admire him love him for his weirdness and wildness and the fact he was a pioneer of electronic music in this country. He was an influential figure in terms of young musicians looking for new directions. And I think he enjoyed the fact that he presented himself as the anti-Christ for a while. Two members of my Inner City Unit band, Nazar Ali Khan and Steve Pond, were huge fans of Genesis P.

Sometime collaborator Douglas Rushkoff, the US cyberspace theorist and graphic novelist, believes Orridge is responsible for bringing the cut-up into the industrial and digital era. Burroughs and Gysin did cut-up manually, with physical newspapers and stuff. Gen did it with music, with genres, and then with industrial noises as a way of reclaiming power. Then he decided to do it with his body, and even with gender. S/he is a living cut-up, and continues to pay the physical, financial and psychological price for it.

Orridge, now resident in New York, insists that its all been worth it: When we do talks and lectures now, weve broken attendance records at universities. It seems to me that my job is to inspire people to take charge of how their life unfolds, to be the author of their own narrative and to throw it away when it gets boring. Its not always easy and it can sometimes feel destructive, but for me its the only way to live.

Successful first year for Somerset Menopause Clinic in Taunton – Somerset County Gazette

MORE than 100 women have visited Somerset Menopause Clinic since it opened a year ago.

Teresa Davison, who runs the clinic, is a nurse practitioner and British Menopause Society Registered Specialist. She believes the success is because the appointments give women time to talk about their symptoms. This can be difficult at a 10 minute doctors appointment.

She said: Since the clinic opened I have seen over 100 women mostly from the South West, not just Taunton and some from as far away as London.

One of the most important things is dispelling the fears about HRT, promoting its safety and actually talking about the positive effects it has. For the majority of women the risks are outweighed by the benefits. This needs time and discussion to evaluate individual needs.

I am thrilled at how successful the business has become and love having the ability to help these women.

Teresa is planning to increase the number of appointments during the week. She offers a 45 minute appointment to discuss the concerns surrounding health issues during the menopause, as well as discussing different treatment options.

She will prescribe hormone replacement therapy (HRT) if appropriate and if the patient decides to try it.

Teresa added: Not every lady needs or wants a prescription. They may just need advice, options and the time to talk.

This is an important service for women going through the menopause - a stage of life that can be unrecognised even by those having symptoms.

On this weeks 51%, why is there so much angst among Gen X women? A female Army recruit talks about her choice, and well hear from a woman who is part of a climate music project.

At age 43, author Ada Calhoun had achieved many of the outward marks of success and personal fulfillment, however, she still felt lousy. In her new book, Why We Cant Sleep: Womens New Midlife Crisis, Calhoun consulted with generational research/ data experts and interviewed hundreds of Generation X women (defined by Calhoun as those born between 1965-1980) in a quest for answers. What she found was that many pervasive struggles expressed by women around her age were unique to Gen X women. By the way, there is a reference during the interview to HRT, which stands for Hormone Replacement Therapy. Calhoun spoke with 51%s Elizabeth Hill about her new book and Gen X women.

The Army has been trying some new methods to recruit more soldiers. A new marketing campaign, called Whats Your Warrior launched in November. Its short on crawling in the mud and long on high tech. Dan Boyce reports for the American Homefront Project.
