The former UFC light heavyweight champion is now signed to the Singapore based ONE organization. This means he is not subject to rigorous USADA testing and TRT could very well be pumping through his veins making the 42 year old move like a man in his early 30s. We are not confirming that he is back on testosterone replacement therapy but he does look better than he did at the end of his last UFC run.

Female-to-male surgery or FTM surgery is a type of sex reassignment surgery, which is also called gender-affirming surgery or gender affirmation surgery. This can take different forms, including a mastectomy, the removal of breasts and the altering of the genital region, also known as bottom surgery.

Examples of bottom surgery include the removal of the uterus known as hysterectomy, removal of the vagina known as vaginectomy, construction of a penis through phalloplasty or metoidioplasty.

Surgery

Before having FTM gender-affirming surgery, a person will receive testosterone replacement therapy. They may then undergo one or more of the following types of procedure.

Chest restructuring

Someone undergoing surgery to transition from female to male usually has a subcutaneous mastectomy to remove breast tissue. The surgeon will also make alterations to the appearance and position of the nipples. Meanwhile, testosterone therapy will stimulate the growth of chest hair.

Removal of the uterus, ovaries, and fallopian tubes

A person may wish to undergo this type of surgery if they are uncomfortable having ovaries, a uterus, or fallopian tubes, or if hormone therapy does not stop menstruation. In a partial hysterectomy, a surgeon will remove only the uterus. In a total hysterectomy, they will also remove the cervix. A bilateral salpingo-oophorectomy or BSO involves the removal of the right and left fallopian tubes and ovaries.

Metoidioplasty

A metoidioplasty is a method of constructing a new penis, also known as neopenis. It involves changing the clitoris into a penis. A person will receive hormone therapy before the surgery in order to enlarge the clitoris for this purpose. During the procedure, the surgeon will also remove the vagina, called a vaginectomy.

Also, they lengthen the urethra and position it through the neopenis. To get the exact length, the surgeon uses tissues from the labia minora, the cheek or other parts of the vagina. The aim of this is to allow the person to urinate while standing.

Another option is a Centurion procedure, which involves repositioning round ligaments under the clitoris in order to increase the girth of the penis.

A metoidioplasty usually takes 2 to 5 hours. After the initial surgery, additional procedures may be needed. A Centurion procedure takes around 2.5 hours and removing the female reproductive organs will add to this time. An advantage of a metoidioplasty is that the neopenis may become erect, because of the erectile abilities of clitoral tissue. However, neopenis resulting from metoidioplasty is usually to small for penetrative sex.

Phalloplasty

A phalloplasty uses grafted skin that is usually from the thigh, arm, abdomen or back, in order to form a neopenis. Doctors consider taking skin from the forearm to be the best option in penile construction. Compared with metoidioplasty, a phalloplasty results in a larger penis. However, this neopenis can't become erect on its own.

After a period of recovery, a person can have a penile implant. This can allow them to get and to maintain erections and have penetrative sex. During a phalloplasty, the surgeon performs a vaginectomy and lengthens the urethra to allow urination through the penis.

The disadvantages of a phalloplasty include the number of surgical visits and revisions that may be needed, as well as the cost, which is usually higher than that of a metoidioplasty.

Scrotoplasty

A person may decide to have a scrotoplasty, which is the creation of a scrotum, alongside a metoidioplasty or phalloplasty. In a scrotoplasty, a surgeon hollows out and repositions the labia majora to form a scrotum and inserts silicone testicular implants.

Kicking off our tenth most popular story was the news that Stelara (ustekinumab), a monoclonal antibody (mAb), had been submitted to the European Medicines Agency (EMA) for approval.

The drug is intended to treat patients with moderate-to-severe ulcerative colitis and is the first mAb to target the interleukin (IL)-12 and IL-23 cytokines, believed to be key in the development of inflammation.

The treatment had previously been submitted in a supplemental Biologics License Application (sBLA) to the US Food and Drug Administration (FDA).

Since January, the drug has been approved for the treatment of the condition for patients who have had an inadequate response with, lost response to or were intolerant to conventional therapy.1

Our next most popular story highlighted the effect of the US government shutdown in January on the FDA. The regulatory body clarified which operations it could carry out during the lack of funding.

According to the FDA, it was able to continue its usual activities that were crucial to ensure public safety in the US.

The statement also emphasised that the agency would guarantee the monitoring and response to emergency situations.

The shutdown ended on 25 January 2019, once President Donald Trump was able to reach a provisional deal with congressional leaders.2

August saw an outbreak of African swine flu in China threaten the supply of a pharmaceutical ingredient to the US.

Stocks of heparin, an ingredient used in anticoagulant drugs and usually derived from pigs, were under pressure as 150 million of Chinas 440 million swine reportedly died as a result of the disease.

Six members of the US Congress wrote a letter to the FDA, outlining their concern for the pharmaceutical supply chain. According to the letter, 60 percent of the crude heparin used to manufacture the finished version of the ingredient in the US is sourced from China.

However, as of October, the FDA reported that the supply of heparin was not affected due to the swine fever.3

In one of many recalls seen this year, the UK Medicines and Healthcare products Regulatory Agency (MHRA) announced an alert in October in regard to Perrigo Companys ranitidine medicines.

The warning came after the discovery of levels of N-nitrosodimethylamine (NDMA), a probable human carcinogen, were found in ranitidine products.

In this case, all unexpired stock of the 150mg/10ml oral solution was recalled and healthcare professionals and retailers were told to cease supply of the medicines.

Since the recall, the EMA has announced that the risk of cancer in humans from the drugs is very low, but that marketing authorisation holders should take precautionary measures to mitigate the risk of nitrosamine formation in medicines.4

In February, the news broke that biologic drugs used to treat severe psoriasis could be used to prevent heart disease in patients with the skin condition. According to a study, after one year of treatment, biologic therapy improved coronary artery plaque, similar to the effect of a low-dose statin.

Systematic inflammation assessed by blood markers also reduced in the group taking biologic therapy, found the researchers.

According to the story, previous research has shown that in heart attack patients, anti-inflammatory biologic therapy reduces the risk of another cardiovascular event.

The study was published in Cardiovascular Research.

In another announcement, the MHRA announced that three batches of Losartan tablets would be recalled in a precautionary measure due to a possible contamination with the nitrosamine N-nitroso-N-methylamino butyric acid (NMBA).

The recall was part of a wider range of sartan medicines affected by potential contamination, a class of drugs to treat blood pressure and heart attacks.

The recent investigation into sartan medicines began in 2018, after NDMA was discovered in valsartan manufactured at a facility based in China.

In August, the FDA released a statement that emphasised the actual risk to patients from sartan medicines is very low and that there are ongoing investigations into the safety of this class of drugs.5

In our fourth most-read story, researchers in Poland and South Korea were reported to have developed a computer model that could be used to synthetically produce essential drugs, bypassing patent-protection.

To overcome the patents in place, the researchers froze the challenging portions from each target molecule, forcing the computer to substitute unconventional but chemically plausible molecules.

The team suggested that their software could help pharmaceutical companies to protect their intellectual properties and increase research into organic chemistry.

The study was published in Cell.

In April, an experimental male oral contraceptive pill called 11-beta-methyl-19-nortestosterone dodecylcarbonate (11-beta-MNTDC) passed human safety tests.

The tablet is formulated with a modified testosterone that has the combined actions of a male hormone (androgen) and a progesterone.

In 40 healthy men, taking the pill was associated with lower testosterone levels, but no side effects were reported, allowing it to pass the test.

According to the researchers, the study indicates that the contraceptive pill decreases sperm production while preserving libido.

The findings were published in The Journal of Clinical Endocrinology and Metabolism.

August saw the story break that the shortage of hormone replacement therapy (HRT) in the UK is likely to continue into 2020. The news led medical experts to call for a resolution to the disruption in supply.

Reportedly, around half of the most commonly prescribed HRT products are out of stock in many pharmacies.

The shortage was due to supply chain issues in China, causing manufacturers to stop production of the therapies. Other brands were consequently in high demand, reducing the availability of most products.

As of November, there were still issues with the HRT supply chain. However, pharmaceutical companies are reportedly addressing these problems and working with manufacturers to solve the issue.6

Our top story this year was in September, when the news broke that the FDA and EMA were investigating some ranitidine medicines after NDMA was detected in some drugs.

Both agencies announced that they would be examining the levels of NDMA found and would take the appropriate measures to protect those taking the medication.

Since then, the FDA has released a statement, after finding that the drugs tested contained no more NDMA than in grilled or smoked meats. However, they also emphasised that manufacturers are encouraged to continue conducting their own testing to ensure the safety of their medicines for patients.7

Through hormone replacement therapy, transgender women can receive estrogen to counteract testosterone and transgender men can receive testosterone to counteract estrogen, said Patricia Kress, the medical director of Student Health Service on campus.

Jorgenson said on campus, students are able to initiate hormone therapy if they have not yet done so prior to coming to campus, or are able to continue hormone therapy if they have already started elsewhere.

Its critical that we support trans people not only in social transitioning but if there are any medical transitions that they want to undertake, he said.

The mental health outcomes are incredible for people who are allowed to take steps to align themselves physically with who they are and how they identify, Jorgenson said.

If they are coming to Student Health Service on campus, visits are completely free, Kress said.

Right now if a transgender male comes in and needs this routine lab work done during a visit, the price is $74, Kress said.

The routine lab consists of checkups six months after the first meeting, 12 months out, and then once a year after that.

For transgender men, clinicians need to test testosterone levels to see if the medicine is working and to keep track of cholesterol, complete blood count, kidney function and blood sugar, she said.

The first year is a little expensive but once you get through that its not so bad, Kress said.

Outside of the university, that $74 panel would easily cost $700 to $1000, so here on campus, the treatment is much more inexpensive compared to other clinics, Kress said.

For transgender females, the blood tests are around $50 because we dont have to do that testosterone level, she said.

For some of these students whose parents cut them off, they have no financial support.

We dont want to make things harder for them by having the treatment be so expensive, Kress said.

The clinicians monitor the students feedback to the dosages very closely, Kress said.

When hormone therapy starts it can be a very exciting time but it also can be rocky with dips in the road, Kress said.

There are counseling services available throughout the process to make sure that the students receiving the treatment feel safe and supported and that the transgender students are doing whats best for them, Kress said.

Its kind of amazing because some students, after you give them that first dosage of the hormone they need, theyre like oh my gosh, a piece of the puzzle has been finally found. Its really powerful, she said. Kress spends a full hour with students at the first appointment, a half-hour at the next two meetings and then sees them in a follow-up within a month, she said.
