In a world first, surgeons at Oregon Health & Science Institute have used the CRISPR gene-editing technique to attempt a cure for Leber congenital amaurosis, a rare genetic condition that causes blindness in early childhood.

While we await results on how this experiment worked out, this achievement joins a list of other medical uses of the technique, including the search for a Huntington's disease cure, herpes, HIV, and immunotherapy for some types of cancer.

Living at a time when medical researchers have this powerful tool at their disposal is certainly a good news item in our books.

In February, a huge stock of 60,000 seed samples was added to the Svalbard Global Seed Vault nestled inside a mountain in Norway's Svalbard archipelago, including the first-ever heirloom seed deposit by an indigenous US tribe.

Researchers have announced that for the second time ever, a patient carrying the HIV virus has been declared cured, with no trace of infection in his blood 30 months after he stopped traditional treatment, undergoing a specialised type of stem cell therapy.

Hydrogen fuel is one of the more promising zero-emissions options around - if only we could produce it cheaply and without needing insane amounts of energy input.

Now, a team of researchers in Tokyo have managed to do just that, refining a method that produces hydrogen fuel using just a few basic ingredients, including light and a particular type of rust. A new study shows this method yields 25 times more hydrogen than existing methods.

Speaking of sustainability, one of the biggest challenges to widespread adoption of renewables remains the problem of large-scale storage. However, there is one excellent solution to this problem - pumped thermal electricity storage. This approach stores electricity by turning it into heat, then turning it back into electricity when needed using an engine.

Unlike pumped hydro, which requires specific geographic requirements, this type of storage can be built in many places, and it uses thermodynamic principles to store electricity in the form of heat. And the best part? It's already being tested in pilot plants.

In June 2019, an odd paper made waves after it was published in Scientific Reports. The scientific community was quick to voice their concerns over this flawed study, which claimed that the Sun's movements were the real cause of anthropogenic global warming.

Davina McCall has revealed shes still taking hormone replacement therapy (HRT) eight years after starting the perimenopause.

The TV presenter, 52, started HRT when she beganexperiencing symptoms like severe night sweats.

Speaking to Sunday Mirrors Notebook magazine, she said: Literallythe first time I put the little magic sticker on I slept through the night.

I hadnt slept through the night for years, Id been lyingon a towel because the sheets were so wet. I felt back to me again. I went fromfeeling desperate to really amazing overnight.

Davina admitted she would stay on the pills until shes 80and even asked the doctor if she could.

He said, Lets just take it year by year, she explained. But at the moment I still feel amazing.

HRT is a treatment to relieve symptoms of the menopause. Itreplaces hormones that are at a lower level as you approach the menopause.

Symptoms of menopause include hot flushes, night sweats,mood swings, vaginal dryness, and reduced sex drive.

The star also recently opened up about her terrifying drug overdose around 30 years ago.

She explained that drugs took her to a very, very, verydark place, and recalled the terrifying night she overdosed.

It was very frightening as it was a very gradual overdose.Id had a cocktail of drugs, because I didnt want to feel normal, sherecalled.

I was constantly trying to escape myself I was like a piece of fruit rotting from the inside.

Speaking at The Sun on Sundays International Womens Day event, she added: So then I thought, Well, Ill just take a valium, and the next thing I knew I woke up in hospital. At my lowest point, when I was filled with self-hatred, it was literally a case of get clean, or die.

Davina went on to say that she began to attend Narcotics Anonymous meetings, and had quit drugs within a year, before mentioning that she still sometimes goes if she feels she needs it.

The extraordinary life and times of Genesis P-Orridge – Louder

In January 1971, 20-year-old Neil Andrew Megson changed his name by deed poll. It was one of the only times we really upset my mum, he recalls. We were hitching down to London and stopped off at my parents place: Erm, theres something we need to tell you. Were not Neil Megson anymore, were now Genesis P-Orridge. And she just started crying. She saw it as a rejection. And it was, but not of her. It was a rejection of the traditional way people labelled and limited their culture.

This rejection of accepted societal norms has been a central theme in the work of Genesis P-Orridge. Its there in the industrial anti-rock of Throbbing Gristle, the band he founded in 1975, and in his subsequent tenure as leader of post-punk deviants, Psychic TV. Both groups thrived on chaos and confrontation, their music a constantly evolving mix of brutal noise, avant-pop and strange electronica. Their lyrical preoccupations too illicit sex, murder, ritual, fascism, the occult were designed to shock, to provoke extreme reactions.

Above all, the idea that identity is fluid rather than fixed is reflected in Genesis P-Orridge himself. If prog is often defined as the impulse to break down barriers and taboos, to explore new opportunities, then he is its living embodiment. Along with second wife Jacqueline Breyer, the 90s saw him begin a series of body modification experiments that involved cross-dressing, breast implants and hormone replacement therapy in order to merge as one pandrogynous entity called Breyer P-Orridge. Another detail was the adoption of neutral gender pronouns (each of them now being addressed as s/he), which also explains why Orridge now refers to himself using the plural we as opposed to the singular I.

Confused? For Genesis, its actually very simple: Theres a phrase that sums it up: Viva la Evolution. I have a patch on the back of my denim jacket that Lady Jaye [aka Breyer] gave me, that just says: Change. Its important for people to understand that they really can be anyone they want to be. Jaye used to say: Why wake up in the morning and be who you wereyesterday?

Lady Jaye died in 2007 from a heart attack, probably related to stomach cancer. Genesis has now documented h/er life, and the couples relationship, in a hefty new book. Genesis Breyer P-Orridge, gleaned from reams of personal archives, is a tome that combines over 350 photographs (some of which, be warned, are very intimate), essays, artwork, commentary and memorabilia.

Its been an extraordinary life. Born in post-war Manchester, h/er musical tastes were initially inspired by h/er father, a Dunkirk veteran with a predilection for jazz. In my early teens, he started taking me to gigs at the Free Trade Hall. We saw Duke Ellington, Count Basie, Buddy Rich, all kinds of amazing jazz musicians. A lot of it was based on improvisation, and that always made sense to me. It was like: Why should it always be the same?

In the 60s, at Solihull School in the Midlands, Orridge discovered the writings of the Beats. H/er imagination was fired by Kerouac, Ginsberg and especially the random cut-up techniques of William Burroughs and Brion Gysin. Then came Dada, Surrealism and Andy Warhol. These peoples lives were as fascinating as what they made, often even more so. It fitted in with how we felt, that it was more like a calling to be an artist of any type. It was like being a priest or doctor. Its a mystical, spiritual activity. Life and art can never be separated.

Then came a fascination with Captain Beefheart, Frank Zappa and Syd Barretts Pink Floyd. After dropping out of Hull University in 1969, s/he joined a semi-commune of freaks holed up in an old warehouse in the city. The university would call whenever they needed anything strange to accompany the many visiting bands who played there.

Orridge in particular remembers Pink Floyd breezing in to tour Ummagumma: We were co-opted as an additional section of the light show. We were so excited at being asked that we ate these hash cookies on the way there, so everyone was stoned and tripping on top. We had these glass slides with liquid in, then also an epidiascope, onto which we put live maggots. So you had this amazing psychedelic light show, with six-foot long maggots crawling across. A lot ofpeople forget Im absolutely a child of the 60s. Throbbing Gristle was hardpsychedelics.

Later that year, inspired by the subversive prankery of Dada and the Situationists, Genesis co-founded the performance art ensemble COUM Transmissions. Alongside Peter Sleazy Christopherson, Chris Carter and Christine Newby (aka Cosey Fanni Tutti), it was the same line-up that would eventually morph into Throbbing Gristle.

At the beginning COUM was basically absurd theatre, s/he explains. People hated us. None of us could play and we just saw it as a big laugh. But as it got more serious, we started homing in on sexual taboos, transgression and who decides whats okay in certain societies. We were seeing how far we could push the envelope of insanity. We had an amplified shop dummy that wed bang with sticks to make rhythms. And one of the singers was on a surfboard on a bucket of water. Cosey was wandering around with a starting pistol, dressed as a schoolgirl. And Sleazy started collaging background tapes of sound.

The transition to Throbbing Gristle coincided with their controversial show Prostitution, held at Londons ICA in 1976. The happening included pornographic images, soiled nappies and an art deco clock stuffed with used tampons under the title: That Time Of The Month. There were questions asked in Parliament. Tory MP Nicholas Fairbairn called the group wreckers of civilisation, a quote gleefully appropriated by TheDaily Mail.

The bands early gigs werent received much better. The first review said: Even an ape with severed arms can play bass better than Genesis. And someone else wrote about my violin: Ive never seen such utter degradation of a beautiful instrument before. We felt encouraged!

With Genesis on vocals, Throbbing Gristle were an uncompromising bunch. Debut LP Second Annual Report was a dense mass of industrial noise that included such queasy listening as Maggot Death and After Cease To Exist, the latter featuring the taped confession of a real-life killer.

For all the disturbing aspects of their work however, it wasnt without black humour and self-mockery. 1978 single United was a favourite in the punk clubs, whereupon TG promptly sped it up as a remixed version that lasted just 16 seconds, while Death Threats, which appeared on that years DOA: The Third And Final Report, was a collage of abusive messages left on their answerphone.

By 1981 though, Orridge had become restless. We were disillusioned and in a rut. TG was becoming commercially viable, but at the same time it was becoming stale. So being an idealist, we just thought it was time to stop.

S/he had been dreaming up a new venture for some time. The sleeve for TGs final single, Discipline, had included the description: Marching Music for Psychick Youth. Plus some of their flyers had carried the words: Psychick Youth Rally!

We were deliberately trying to sow the seeds of Psychic TV before the end of TG, s/he admits. We wanted to do something that was much more positive. Its alright to be negative, like TG could be, and say everything sucks. But after a while you have to say: Well what are you gonna do about it? So the idea was for Psychic TV to not just be a band, but also a network where the people who would normally just be fans are in more direct touch. And it grew into this alternative way of doing culture.

Co-founded by Orridge and Alternative TVs Alex Fergusson, with Peter Christopherson joining soon after, Psychic TV were far more free-ranging than TG. There was still a fair spread of juddering mechanical noise, but also a raw minimalism that allowed room for electronic pop, day-glo psychedelia and the liberal use of samples. By 1988 they were even at the forefront of the rave explosion with the single Tune In (Turn On The Acid House) and LP, Jack The Tab.

There were more diverse musical wanderings up until the late 90s, when Orridge decided to call it quits and concentrate on spoken-word projects. He revived Psychic TV for 2007s Hell Is Invisible Heaven Is Her/e and follow-up Mr Alien Brain vs. The Skinwalkers.

The bands present compass points to all things prog. In the early 70s, Orridge devoured Amon Duul, Can, Jan Dukes de Grey and Hawkwind. Prog had improvisation, which was interesting. And weve rediscovered it now with Psychic TV. Were doing a series of 12-inch singles. The first one was Funkadelics Maggot Brain, but we added vocals. Then we did Mother Sky by Can, Silver Machine and Hurry On Sundown by Hawkwind.

S/he finally got to play with Hawkwinds Nik Turner in San Francisco in 1996. Genesis is a very revolutionary character, offers Turner. Psychic TV and Throbbing Gristle were vehicles for his off-the-wall expressionism. Those who admire him love him for his weirdness and wildness and the fact he was a pioneer of electronic music in this country. He was an influential figure in terms of young musicians looking for new directions. And I think he enjoyed the fact that he presented himself as the anti-Christ for a while. Two members of my Inner City Unit band, Nazar Ali Khan and Steve Pond, were huge fans of Genesis P.

Sometime collaborator Douglas Rushkoff, the US cyberspace theorist and graphic novelist, believes Orridge is responsible for bringing the cut-up into the industrial and digital era. Burroughs and Gysin did cut-up manually, with physical newspapers and stuff. Gen did it with music, with genres, and then with industrial noises as a way of reclaiming power. Then he decided to do it with his body, and even with gender. S/he is a living cut-up, and continues to pay the physical, financial and psychological price for it.

Orridge, now resident in New York, insists that its all been worth it: When we do talks and lectures now, weve broken attendance records at universities. It seems to me that my job is to inspire people to take charge of how their life unfolds, to be the author of their own narrative and to throw it away when it gets boring. Its not always easy and it can sometimes feel destructive, but for me its the only way to live.

Successful first year for Somerset Menopause Clinic in Taunton – Somerset County Gazette

MORE than 100 women have visited Somerset Menopause Clinic since it opened a year ago.

Teresa Davison, who runs the clinic, is a nurse practitioner and British Menopause Society Registered Specialist. She believes the success is because the appointments give women time to talk about their symptoms. This can be difficult at a 10 minute doctors appointment.

She said: Since the clinic opened I have seen over 100 women mostly from the South West, not just Taunton and some from as far away as London.

One of the most important things is dispelling the fears about HRT, promoting its safety and actually talking about the positive effects it has. For the majority of women the risks are outweighed by the benefits. This needs time and discussion to evaluate individual needs.

I am thrilled at how successful the business has become and love having the ability to help these women.

Teresa is planning to increase the number of appointments during the week. She offers a 45 minute appointment to discuss the concerns surrounding health issues during the menopause, as well as discussing different treatment options.

She will prescribe hormone replacement therapy (HRT) if appropriate and if the patient decides to try it.

Teresa added: Not every lady needs or wants a prescription. They may just need advice, options and the time to talk.

This is an important service for women going through the menopause - a stage of life that can be unrecognised even by those having symptoms.

Definite Wellness is bringing a whole new meaning of health to residents of Tullahoma and surrounding areas. Owner Candi Kinney has been a nurse practitioner for three years. She graduated from MTSU and was previously an RN for 3 years.

When I was a nurse, I worked at a hospital. I did not like the concept that patients would come in really sick and all we would do was put a Band-Aid on them and send them home fast. I did not like the fact that some patients were so chronically ill, so I decided wellness care was a better fit for me, Kinney said. I want to make it to where patients can come in here, address the issues they have going on and we stop it from getting to that point where it becomes chronic.

The office, located inside of the Coker building, houses two exam rooms, an office, and a main room with a front desk and chair where intravenous (IV) therapy is administered.

Kinney offers eight total signature IVs including the energizer, the glow, the fighter, the quencher, the athlete, the classic, the Candi cocktail and the morning after.

People can also come in and create their own IV by looking at the menu and all that I have to offer, she said. IV add-ons include B12, Calcium, Zinc, Mag-nesium, Vitamin C, B-Complex and Glutathione for an extra charge.

She also offers injections including B-12, lipo shots, Toradol, Vitamin D and Zofran.

Benefits of wellness care

I am all about things that make and keep you well. When you come in and get a B-12 or Vitamin D shot, that is going to keep you well because when either one of those is low, it can cause a lot of problems. That is what got me started with wellness care, Kinney said.

What got me started in IVs was the interest that people have in them today. This trend is huge out west. There are IV bars everywhere and I thought This is really good for recovery, athletes, dehydration and more. It is so interesting that this is something I can do, Kinney said. I signed up for an IV hydration class and decided to start offering it.

I had a man call me the other day who was so sick and asked if I could come to his house and give him an IV. I went to his house and gave him two bags of IV fluids and he texted me the next day telling me how much better he felt, Kinney said. It is also good for athletes, especially those who are into CrossFit or running marathons. Coming here is a good way to prepare your body for that and it is also a great way to recover from them. One of my friends came in and told me that she was so stressed out and wiped and needed some help, so I gave her an IV. A few hours ago, she said she felt fantastic, she added.

These IVs are not just for running marathons. There are a lot of benefits if you are big on working out. I am a former football player, so every spring I start working out again. I was very skeptical of the IV trend, Kinneys husband and business partner Eugene said. I was telling Candi how sore I was from a workout and asked her if she had anything to help me. She gave me an IV called the athlete. After I got it, I felt okay but was not that impressed. However, the next morning I was so surprised to be up and ready to go workout again.

As a person who has struggled with weight my whole life, I do not believe in easy, quick fast out there diets. I can put anyone on a low-calorie diet and give them HCG shots and they will lose weight, but it will not be sustainable, Kinney said. My patients need to have a sustainable lifestyle. I advocate a whole food diet, 30 minutes of exercise and 60 ounces of water per day. If you do that in conjunction with what I offer, it is going to get you to your goal weight. However, it is up to you to maintain it.

With weight loss, I do a couple of different things. I offer three different weight loss programs, Kinney said. I distribute phentermine here in the clinic so it is like a one-stop shop.

The first option, Tier 1, is a 12-week program that includes one visit each month with Kinney, a 30-day supply of phentermine if qualified, six bi-weekly fat-burning lipo shots and one Slim IV each month, totaling at $600.

Tier 2 is a 12-week program that includes one visit each month with Kinney, a 30-day supply of phentermine if qualified and six bi-weekly fat-burning lipo shots, totaling at $285.

I usually do not treat patients under the age of 18, Kinney said. If you are 16 or older and you come in with a parent, I can treat you.

When a customer comes in, they are instructed to fill out paperwork and Kinney reviews the forms with them. If there is no conflicting medical history or a contraindication, Kinney administers the IV the same day. There is no additional charge for coming in to receive an IV. A customer will only be charged for how much the IV costs. She also does sport, D.O.T. physicals and walk-in sick visits that do require a $75 fee.

I treat simple things like ear infections, UTIs, sore throat, flu and strep for much cheaper than an urgent care visit would be, she said.

Kinney also offers bioidentical hormone replacement therapy. This is typically for middle-aged people for replacements of estrogen, progesterone and testosterone, she said.

This is one of those shops where people need this, they just do not know that the services are here, she said.

The contagious coronavirus can survive on a cardboard delivery box for at least a day and lives even longer on steel and plastic.

A big question in the outbreak of Covid-19, which has already infected more than 110,000, is how the germ that causes it moves so easily between people. Although many viruses and germs can survive on ordinary objects, zeroing in on precisely how the new coronavirus does it could help stem the epidemic.

To help find an answer, US researchers tried spritzing the virus on seven materials commonly found in homes and hospitals, to see how long it remained infectious.

The germ survived longest on plastic and stainless steel, where it clung for as long as three days, according to Vincent Munster and a team at the National Institutes of Health virology laboratory in Hamilton, Montana, who describe their experiments in a new preprint.

That suggests hospital equipment is a potential vector for disease, as are strap-hanger poles on subways.

So far, however, there is no definitive proof the virus is actually spread via inanimate objects. We dont know if you can pick up Covid-19 from contaminated surfaces or inanimate objects at this point. Thats the bottom line, says Marilyn Roberts, a microbiologist at the University of Washington School of Public Health.

Doctors testing patients know that the virus is present in large amounts in peoples upper respiratory tract, making it likely that it gets spread when they cough or sneeze, spraying tiny droplets and aerosols into the air and onto surfaces.

Virus stability in air and on surfaces may directly affect virus transmission, as virus particles need to remain viable long enough after being expelled from the host to be taken up by a novel host, Munster and his team write.

Munster and his coworkers say spreading via the air likely explains super spreader events like one that appears to have occurred in Boston, where more than 70 people are believed to have been infected at a conference held by the biotechnology company Biogen.

The scientists looked at how long the virus lived on different materials, and also as it swirled in an air chamber. After waiting a few hours or days, they wiped the surfaces and checked to see if they could still infect cells in a petri dish.

Materials the virus liked best were stainless steel and plastic, where infectious germs could still be collected after three days and might endure quite a bit longer. It liked copper least: the virus was gone after just four hours. Swished around in the air chamber, the germs remained for about three hours.

It will take detailed epidemiological studies to find out for sure exactly how the virus spreads, but the new lab findings indicate it can at least cling to cardboard boxes such as Amazon packages, or plastic cell-phone cases.

With some viruses, like influenza, touching a surface can sweep up millions of viral particles in just a few seconds. Other studies show that people touch their faces more than 20 times every hour. Cold temperature and low humidity let viruses live even longer.

Health authorities are recommending that people wash their hands frequently and use alcohol-based cleansers to disinfect surfaces. Its known that coronaviruses are fairly easy to kill; rubbing alcohol and diluted hydrogen peroxide are among the effective weapons.

In a February review of whats already known about this type of virus, German researchers said that within a minute of cleaning a surface, a million viral particles can be reduced to 100, likely reducing the risk of infection.

The NIH researchers compared the new virus with SARS and found that the two germs stuck around for similar lengths of time. SARS caused an outbreak in 2003 but was not so easily transmitted, which means other factors play a role in why Covid-19 is spreading faster.

While those reasons arent fully understood, the NIH researchers speculate that people with the new coronavirus could be shedding it even if they dont have symptoms or that that it takes less of the virus for a person to become infected, a metric known as the infectious dose.

The researchers say they are now looking at how long the virus lasts in snot, spit, and fecal matter, and under what temperatures and humidity levels.

Ethiopian investigators have concluded that the March 2019 crash of an Ethiopian Airlines flight was caused by design flaws in the Boeing 737 Max plane and not by the performance of the airline or its pilots, adding to the scrutiny of the jet model that has been involved in two recent deadly crashes. An interim report released Monday by the Ethiopian Aircraft Accident Investigation Bureau comes almost exactly a year after Ethiopian Airlines Flight 302 went down shortly after departing Addis Ababa, killing all 157 people on board. The crash occurred five months after a similar Max owned by Lion Air of Indonesia crashed minutes after takeoff, killing 189 people. Although several factors have been cited in the two crashes, malfunctions related to automated software known as MCAS were listed as key in both accidents. A Boeing spokesman said Monday that the company continued to extend our heartfelt sympathies to the families and loved ones of Ethiopian Airlines Flight 302. The Indonesian report cited a number of factors, including aircraft design, the flight crews response and a lack documentation on the planes flight and maintenance history. In Ethiopias case, investigators found that the aircraft had a valid certificate of airworthiness, had no known technical problems before departure, and had weight and balance within the operating limits. But they said faulty sensor readings and automatic commands that did not appear on the flight crew operation manual had left the crew unable to control the plane, resulting in the fatal crash. The worlds wealthiest family moved $48 billion of Walmart Inc. stock to a different holding company in an action that may signal future share sales and bolster their philanthropy. Walton Enterprises LLC, the main investment entity of the retailers founding clan, transferred about 15 percent of Walmarts outstanding shares to the Walton Family Holdings Trust, according to regulatory filings last week. Aon agreed to buy Willis Towers Watson in an almost $30 billion transaction that combines the worlds second- and third-biggest insurance brokerages. Increase in Younger Bowel Cancer Cases Highest in Southern England – Medscape

The increased incidence of colorectal cancer (CRC) in younger adults is not simply a demographic shift but reflects potential biological changes in the disease as well as alterations in geographical distribution that need to be better understood, say UK researchers in a large population-based study.

Numerous recent studies from the US, as well as those in countries across Europe, Australia, New Zealand, and Canada, have shown that there has been an increase in CRC incidence in younger people in the past few decades.

As reported by Medscape Medical News, these studies suggest that, while the overall incidence of CRC may have stabilised, the incidence in adults aged under 50 years has risen sharply, at rates ranging from 1.5% to 8% per year.

Seeking to provide a more detailed picture of the shift, Adam Chambers, School of Cellular and Molecular Medicine, University of Bristol, and colleagues looked at data on more than 56,000 UK adults aged 2049 years diagnosed with the disease between 1974 and 2015.

The new research, published online by the British Journal of Surgery, showed that, following an initial dip in incidence, rates increased initially in adults aged 2029 years, followed by those aged 3039 years, at rates of up to 6% per year.

While gender and socioeconomic status did not appear to have an impact on the change in incidence rates, there were notable geographic variations, with the largest increases in southern England, and distal tumours were found to be the biggest driver of new cases.

Mr Chambers commented in a news release: "Age has always been a major risk factor for bowel cancer, with the majority of cases being diagnosed in patients over 60 and therefore bowel cancer screening has focused on older age groups.

"However, this study shows that over the past 30 years, there has been an exponential increase in the incidence of bowel cancer among adults under 50."

Co-author David Messenger, a consultant colorectal surgeon at Bristol Royal Infirmary, added: "Future research needs to focus on understanding why this trend is occurring and how it might be reversed, potentially through the development of cost-effective testing strategies that detect tumours at an earlier stage or polyps before they become cancerous."

Mr Messenger told Medscape News UK that, while this is a population-based study and cannot demonstrate cause and effect, he believes that there are likely to be multiple factors underlying the trend that are "not necessarily the same" for different countries or regions.

He believes that "undoubtedly some of it is dietary related and some of it also obesity related" but, unlike in, say, lung cancer, it is "much more difficult to pick apart" the relationship between lifestyle factors and the development of CRC.

He said that, "based on when weve seen the rise in incidence, it is probably something to do with how our lifestyles have changed really from the mid-60s onwards" because, for successive generations from that period, "youve got a cohort effect" of increasing incidence.

Mr Messenger also notes that the shift in tumour location and the shift in geographical distribution of cases suggests that "the biology of the disease in young adults is different", which could have healthcare implications if the increase in CRC incidence in younger patients continues.

The authors point out that, while increases in rates of CRC have been observed in younger adults across Europe and North America, there appear to be differences in the way the disease manifests in younger versus older populations.

They note that it therefore is "vital" that the epidemiology underlying the increase is better understood, "as young adults typically present with more advanced tumours that carry a poorer prognosis".

To investigate further, the team examined data from the UK-wide National Cancer Registration and Analysis Service database on all adults aged 2049 years diagnosed with CRC between 1974 and 2015.

They also obtained population estimates from the Office for National Statistics and the European Standard Population report to examine trends in age-specific incidence rates, stratified by sex, anatomic subsite, Index of Multiple Deprivation quintile, and geographical region.

The researchers identified a total of 1,145,639 new cases of CRC diagnosed during the study period, of which 2594 were in adults aged 2029 years, 11,406 among 3039 year olds, and 42,314 in those aged 4049 years.

Joinpoint regression analysis revealed that after an initial decrease, there was a notable increase in cases among individuals aged 2029 years, at an annual percentage change (APC) of 4.6% in women from 1986 and 5.1% in men from 1992.

In adults aged 3039 years, the increase started later, at an APC in women of 3.8% from 1995 and 6.0% in men from 2002.

The increases were smaller in adults aged 4049 years and started later, at an APC of 1.5% in women and 0.8% in men, beginning in 2003.

These results, the team writes, are "suggestive of an age cohort effect".

Looking at the data in more detail, they found that the incidence of proximal CRCs, primarily driven by caecal and ascending colon cancers, was increased in 2029 year olds, at an APC of 4.4% from 1995, and in 3039 year olds, at an APC of 5.8% from 2005, but not in 4049 year olds.

However, the increase in incidence rates for distal cancers was higher and more sustained, at an APC of 5.6% from 1991 for 2029 year olds, and an APC of 3.3% from 1995 to 2006 and 7.0% from 2006 for adults aged 3039 years. For those aged 4049 years, the APC was 1.4% from 2001.

While there were few differences noted when stratifying individuals by socioeconomic status, there were differences observed when the researchers looked at geographical region.

For example, incidence rates of proximal CRC among 2049 year olds were, in 1985, decreasing all across England apart from in London, with the South West recording an APC of -12.1%.

By 2015, however, incidence rates for proximal CRC were increasing fastest in the South East at an APC of 7.4%, in London by 6.5% per annum, and in the East at an APC of 6.0%.

The increase was even greater for distal cancers, with all southern regions showing annual increases in incidence rates of more than 5%, rising to an APC of 10.1% in the South West.

"It is difficult to explain why incidence rates are increasing more rapidly in young adults in the south given that risk factors such as obesity are increasing faster in northern regions," the team says.

While acknowledging there may be issues around access to healthcare at play, they add: "The role of environmental factors, such as diet, obesity, physical exercise and the gut microbiota, in the development of youngonset colorectal cancer is incompletely understood and requires further research."

Mr Messenger said the evidence nevertheless suggests that the biology of CRC in younger adults differs from that in their older counterparts.

"That then begs the question about what is happening in the UK that were seeing such pronounced increases in southern regions," particularly in terms of the dramatic increase in CRC cases in the distal large bowel.

He highlighted that, "typically, people have thought this is maybe a disease that is perhaps related to lower socioeconomic groups but weve not seen that in our study the increased rates have occurred equally across all socioeconomic groups in the younger adult age group".

However, Mr Messenger noted that, over time, there has been northsouth migration in England, as well as "increasing urbanisation in southern regions, particularly the South West".

He pointed to cities such as Bristol and towns along the M4 corridor, including Swindon, which are increasing in size and becoming more and more urbanised. In contrast, Middlesborough and Teeside are depopulating, a phenomenon he ascribes to internal migration.

Mr Messenger added that, as well as tumours in younger adults having a "greater propensity to be in the sigmoid and the rectum, theres some evidence that molecularly, they are slightly different; they dont necessarily exhibit microsatellite instability in the same way that you see with older adults".

He said that this observation cannot currently be fully explained, and is the focus of future research. There is also evidence to suggest that younger adults are more likely to present with metastatic or locally advanced disease.

He believes that screening could therefore help with identifying patients at an earlier stage, although not via the blanket use of flexible sigmoidoscopies or colonoscopies but rather greater application of faecal immunochemical testing, which is currently being used in the over-50s.

"The question is do we then roll that out to adults under 50 as a means of risk stratifying those groups, as to whether or not they should have endoscopic examination," Mr Messenger asked.

This would also be a way of obtaining more data on whether young adults really do present with more advanced disease.

"Weve got some idea that they might do worse because theyve got more advanced disease but that is really not set in stone and there is virtually no data on that in the UK, so thats really where things will be heading," he said.

If the current trends continue unchecked, Mr Messenger believes that the types of cases "that we see in 20 years time will look very different".

"At the moment, we think that about 5% of all bowel cancer occurs in adults under 50 but if you were to extrapolate out those rates of increase out to 2041we think it could account for anywhere from about 8% up to 30% of all cases, so youve got the potential in 20 years that patients with bowel cancer could be a very different group."

He continued: "This is anecdotal but Im seeing a lot more younger adults with advanced disease.

"So overall, globally in the population, probably the incidence will trickle down and everyone will think thats great, but actually if you look at it more deeply, those that are getting the cancers will be different. It will be younger adults with more advanced disease."

Mr Messenger said: "Suddenly were having a paradigm shift from where this is seen as a disease of middle age and elderly to then becoming a disease of a younger population."
