A Natural Cure for Cancer?: THE FUTURIST Interviews Dr …

When people come down with cancer, they submit to regimens of drugs and, if need be, harsh radiation treatments. But what if those patients could forego all of this and ward off their cancers with their own white blood cells? An experimental white-blood-cell-transfusion approach that Dr. Dipnarine Maharaj is developing might make that feasible.

Dr. Maharaj is a hematologist and oncologist at the South Florida Bone Marrow/Stem Cell Transplant Institute, a cancer treatment center that applies stem-cell therapies to cancers that have not responded well to other treatments. He is working on taking white blood cells from healthy donors and fusing them into patients with cancer, so that the transfused cells can stimulate the patients immune systems and enable them to ward off the cancers on their own.

His concept has precedentsdoctors successfully treat some other types of infections by transfusing white blood cellsand the initial experimental results are promising. He will need more time, however, and much more funding before his treatment approach is ready. Dr. Maharaj described his research to Rick Docksai, associate editor for THE FUTURIST, in the following interview.

THE FUTURIST: Strengthening the body to wage its own fight against cancer, instead of relying on drugs or radiation, is certainly an appealing idea. What first drew you to it?

Dipnarine Maharaj: We asked the question, why is that some people get cancer and others dont? The answer is that someone whos got cancer, their immune system is broken down. So if the people who didnt get cancer, their immune systems are not broken down, how can we fix the cancer patients immune systems? Im a stem-cell physician, and weve had this procedure for many years where we use a patients own stem cells or the stem cells of a donor to reform the patients own immune system. Thats what actually helps to cure the cancer.

THE FUTURIST: How does your new approach go about boosting the bodys immune cells? What mechanisms are involved?

Maharaj: To cure cancer, we really have to repair the immune system. What were trying to do is apply that same knowledge to treat patients with solid tumors. The method Im using, were taking cells of the immune system from the donors, and were transfusing those cells into patients who have cancers. It is essentially a white-blood-cell transplant.

THE FUTURIST: How early in the progression of cancer would a patient need to be for the treatment to work effectively?

Maharaj: Were still under the clinical trials. But the best way I could answer that question is that the smaller the amount of disease at the time that it is done, the better the chance of a positive outcome.

The University of Michigan is a long-time leader in the study of stem cells. So when voters in Michigan passed a 2008 state constitutional amendment allowing the production of new embryonic stem cells, the University was poised to quickly become a leader in this emerging and potent field of medical research. With the formation of the Consortium for Stem Cell Therapies (CSCT) in early 2009, the University created a multi-disciplinary team of leading researchers from across campus. As part of the U-M Medical School, the CSCT became responsible for the derivation, storage and distribution of new induced pluripotent stem cells and human embryonic stem cells.

Today, the division of the CSCT working with human embryonic stem cells is known as MStem Cell Laboratories. MStem Cell is one of only a handful of academic institution-based programs in the country dedicated to the derivation of disease affected stem cells. The MStem Cell Laboratories mission is to produce unique stem cells that will accelerate our understanding of specific human disorders and lead to eventual therapies and cures. The development of innovative technologies for the growth of stem cells is at the forefront of efforts. Bringing the global research community the best tools to improve future study potential and shorten the time span from initial discovery to clinical application is the goal.

A lot has been accomplished in the last few years. Researchers at MStem Cell Laboratories have already produced ten genetically normal and 17 disease-affected embryonic stem cell lines, providing new and invaluable resources to scientists at the U-M and throughout the global research community.

In this emerging field of medical research and clinical care, MStem Cell Laboratories are working on innovations to more rapidly move new stem cell therapies through the human trials process, and on to clinic use. In an exciting collaboration with chemical engineers and material scientists in the U-M College of Engineering, they have developed a new fully-synthetic artificial surface for the growth of stem cellsthe first of its kindwhich prevents tissue contamination from foreign feeder cells. Until now, mouse feeder cells have been the growth surface for stem cells. This non-human cell-cell interaction with human stem cells contaminates the human cells with mouse proteins and has meant a mandatory fifteen-year wait period between Phase I (safety) and Phase II (efficacy) human trials investigating stem cell replacement therapies. The new synthetic or xeno-free growth surface was designed to eliminate that lengthy gap.

Stem Cells Hashimotos Thyroiditis Hashimotos thyroiditisorchronic lymphocytic thyroiditisis an autoimmune diseasein which the thyroid gland is gradually destroyed by a variety of cell- and antibody-mediated immune processes. It was the first disease to be recognized as an autoimmune disease.It was first described by theJapanese specialistHashimoto HakaruinGermanyin 1912.

Hashimotos thyroiditis very often results inhypothyroidismwith bouts ofhyperthyroidism.

Physiologically,thyroid peroxidaseand/orthyroglobulincause gradual destruction offolliclesin thethyroidgland. Accordingly, the disease can be detected clinically by looking for these antibodies in the blood. It is also characterized by invasion of the thyroid tissue byleukocytes, mainlyT-lymphocytes. It is associated withnon-Hodgkin lymphoma.

Symptoms of Hashimotos thyroiditis includeMyxedematous psychosis, weight gain,depression,mania, sensitivity to heat and cold,paresthesia, fatigue, panic attacks,bradycardia,tachycardia, high cholesterol,reactive hypoglycemia, constipation, migraines, muscle weakness, cramps, memory loss, infertility, and hair loss.

The thyroid gland may become firm, large, andlobulatedin Hashimotos thyroiditis, but changes in the thyroid can also be nonpalpable. Enlargement of the thyroid is due tolymphocytic infiltrationand fibrosisrather than tissuehypertrophy.

Hashimotos thyroiditis is often misdiagnosed asdepression,cyclothymia,PMS,chronic fatigue syndrome,fibromyalgiaand, less frequently, as ananxiety disorder. Testing forthyroid-stimulating hormone(TSH), Free T3, Free T4, and the anti-thyroid antibodies (Tg) and thyroid peroxidase (TPO) tests can resolve any diagnostic difficulty.

Hashimotos when presenting as mania is known as Prasads syndrome after Ashok Prasad, the psychiatrist who first described it.

Risk factors: A family history of thyroid disorders is common, with theHLA-DR5gene most strongly implicated conferring arelative riskof 3 in the UK. In addition Hashimotos thyroiditis may be associated withCTLA-4gene since the CTLA-4 antigen acts as an inhibitor to T-Cell activation only if the red blood cells have an RH factor of negative 3.25.

Preventable environmental factors, including high iodine intake, selenium deficiency, and pollutants such as tobacco smoke, as well as infectious diseases and certain drugs, have been implicated in the development of autoimmune thyroid disease in genetically predisposed individuals.The genes implicated vary in different ethnic groups and the incidence is increased in patients with chromosomal disorders, includingTurner,Downs, andKlinefelters syndromesusually associated with autoantibodies against thyroglobulin and thyroperoxidase.
