Promise of mobile technology provided to hospitalists not impacting patient care – EurekAlert

INDIANAPOLIS --As hospitalists physicians who providegeneral medical care to hospitalized patients become increasingly common, a new study provides a better understanding of both missed and potential opportunities for integrating mobile technology, such as cell phones, tablets and applications (apps) into hospitalists' workflows to improve patient care.

Healthcare systems across the country have invested heavily in health information technology [HIT], but when it comes to the mobile technology provided to hospitalists, HIT hasnt incorporated many user-centered designs, said study corresponding authorand human factors engineer April Savoy, PhD, a Regenstrief Institute and U.S. Department of Veterans Affairs researcher and a Purdue School of Engineering and Technology at IUPUI faculty member.Cell phones work well for ordering a ride form a car service or posting social media because their apps are designed for the consumers these companies wish to reach. But we found a lack of user-focused mobile technology apps designed todecrease difficulty and increase efficiency of hospitalists' workflows and improve patient care.

We determined that when hospitalists leave their desks to see patients, these hard-working physicians are relying upon paper, pen, and most importantly, their memory, Dr. Savoy noted.This reliance on memory contributes to the cognitive load of daily work and can threaten patient safety. With the increasing volume and acuity of patients everywhere, ways to improve day to day care in the hospital are urgently needed.

During rounds, hospitalists dont use mobile technology because its not user focused. It lacks apps that are easy touseon small screens without keyboards because most healthcare systems havent figured out how to make mobile devices accessible when and where needed and havent solved such basic issues like battery life and device sanitation.

After mapping out workflows and interviewing hospitalists, the study authors identified tasks that were most frequent, redundant or most difficult from the providers perspective and posited potential solutions including types of apps that could be prioritized to address issues and provide solutions. Based on their analysis they concluded that mobile apps focused on data entry and presentation supporting specific tasks should also be prioritized.

Clinician perspectives on unmet needs for mobile technology among hospitalists: workflow analysis based on semi-structured interviews is published online in JMIR Human Factors. The study, which was funded by the Department of Veterans Affairs, was conducted toinform the design and implementation processes of future health information technologies to improve continuity in hospital-based medicine and ultimately patient care.

Jake’s mice: Searching for answers to the puzzle of autism – huntingdondailynews.com

ST. LOUIS (AP) Jake Litvag leaned in for a closer look as a lab mouse scurried around an enclosure, stopping to sniff a large block.

Hi, Jakob 1. Im Jake, the 16-year-old said, naming the little furry creature engineered to have the same genetic abnormality he has.

That mouse and its lab-grown relatives are the first in the world to mirror the missing gene that causes Jakes autism. Scientists at Washington University in St. Louis bred the mice, and grew stem cells derived from Jakes blood, to study and find ways to treat his rare disorder and look for answers to the larger puzzle of autism.

Jakes family raised money for the early research, which scientists then parlayed into a $4 million grant from the National Institutes of Health to delve more deeply into Jakes gene, one of more than 100 implicated in autism. They hope to find points of convergence that could someday help people with all forms of the neurodevelopmental condition affecting one in 44 U.S. children.

Jake knows he inspired their work. And thats helped him see autism as something to be proud of rather than something that makes him different from other kids. His parents, Joe and Lisa Litvag, figured meeting the scientists and the mice would show him firsthand what he had brought into being.

Oh wow. Cool! Jake said as he watched a mouse climb down a pole while others scampered in a bin.

Walking out of the lab, tears welled up in Lisa Litvags eyes as she thought about the language within her sons cells helping other kids.

Were deeply proud and humbled to be part of this, said Joe Litvag. What do we live this life for? Its ultimately to try to, in one way, shape or form, be of service to others.

JAKES GIFT

The Litvags realized early on that Jake wasnt reaching childhood milestones. He couldnt walk without assistance until he was 4. He struggled to string sentences together in first grade.

At first, no one could pin down why. Jake had a mix of different traits. He was hyperactive and impulsive but also social, warm and funny. It took until he was 5 to get a firm diagnosis of autism.

Around that time, the Litvags heard that child psychiatrist Dr. John Constantino, an expert on the genetic underpinnings of autism, was giving a talk at the Saint Louis Science Center. They decided to go in the hopes of meeting him. They did, and he began seeing Jake as a patient.

About five years later, Constantino proposed genetic testing. It revealed the missing copy of the MYT1L gene believed to cause one out of every 10,000 to 50,000 autism cases. Having an extra copy can cause schizophrenia.

The finding brought the family peace. Theyd heard lots of people say autism was mostly caused by external factors, like birth trauma. For a long time, Lisa Litvag said, I thought it was something that I did.

Actually, a large multinational study suggests that up to 80% of the risk for autism can be traced to inherited genes.

One of the big things it did for us as a family is it made us realize that its nothing that we did wrong, Joe Litvag said. Its just that people are born all the time with genetic differences.

The couple, whose younger son Jordan doesnt have the condition, talked openly with Jake about his autism and tried to bolster his self-esteem when he worried about being seen as different. They sent him to a small private school that tailors its curriculum to each childs learning abilities. And they encouraged his social tendencies, cheering him on when he and some classmates formed a band, the Snakes.

We never wanted him to feel there was shame around his diagnosis, Lisa Litvag said. We continued to kind of reinforce that this is a superpower, you are special, you are awesome and because you have autism, there are gifts you have to give other people.

GIFTS BLOSSOM

When Constantino suggested studying the little-understood MYT1L gene, the Litvags enthusiastically agreed to help. Constantino who is on the local board of a group theyve long been active in called Autism Speaks asked if theyd be interested in raising money for early research.

Joe Litvag, an executive in the live music industry, and Lisa Litvag, a partner in a marketing firm, reached out to family and friends and raised the $70,000 needed in about six months.

With half the money, researcher Kristen Kroll and her team reprogrammed cells from Jakes blood into induced pluripotent stem cells, which can be prodded into becoming various cell types. With the other half, scientist Joseph Dougherty and his team followed the blueprint of Jakes genome and induced his mutation in mice using the gene-editing tool CRISPR.

Like the people theyre meant to model, mice with the mutation tended to be more hyperactive than siblings without it, running around their cages much more. They were nonetheless generally heavier, especially the first generation of mice. They had slightly smaller brains and a little less of the white matter that speeds communication between different brain regions.

Since starting the research about three years ago, scientists have bred around 100 mice with Jakes mutation and are now using the great-great grandchildren of the first one they engineered. They recently published about the mice in the journal Neuron.

While scientists cant go back and see how Jakes brain developed, Dougherty said, mice allow them to watch the mutation play out through generations.

A GIFT IN RETURN

Dougherty and his colleagues hope what they learn about how MYT1L functions ultimately leads to medicines or gene therapies that improve or even correct the problems the mutation causes.

They are sharing their findings with scientists studying other autism-causing genes or trying to figure out how various genes work together to cause the condition. According to the Simons Foundation Autism Research Initiative, more than 100 genes have strong evidence linking them to autism and a growing list contains several hundred more genes thought to be linked to the condition.

In cases where autism is caused by a single gene, Dougherty said that gene probably does many things to brain development. A key to understanding autism overall is to find one or two things shared across different forms of autism - which could then be targets for treatment. Though not everyone with autism wants treatment, Dougherty said it could help those who do.

Since the research began, Dougherty has been writing notes to the Litvags explaining the latest discoveries. But as a lab scientist, hes mostly removed from the people sparking the research and first met the family when they were invited by the school to visit in December.

After meeting the mice, they stopped into another lab, where Jake peered through a microscope at his blue-stained stem cells.

Thats me! Thats cool stuff. I never saw anything like that in my life, he said, stepping back to lean into his dad, who pulled him close.

Dougherty used the visit as an opportunity to share some news, a gift of sorts that he wanted to tell the family in person.

The missing gene doesnt seem to shorten life. The mice live 2 to 3 years, the same as their siblings.

So, a normal life span? Joe Litvag asked hopefully.

Yes, Dougherty answered. As far as we can tell, identical. I know thats a big relief, too.

Joe Litvag turned to his son. So Jake, maybe you will live to be 100.

I will be 112! Jake replied with a grin.

Realizing the vision of culturing organs for use in life-saving transplantation procedures is still a long way off. However, the work of Prof. Jacob Hanna on stem cells is paving the way for this to become a reality.

Hanna and his team from the Weizmann Institute of Sciences Molecular Genetics Department have found a way to culture human stem cells in a much earlier state than was previously possible. Not only that, the stem cells they created are far more competent, meaning that they are able to integrate more efficiently with their host environment. This substantially improves the chances of obtaining what is called a cross-species chimera allowing cells from one creature to play a substantial role in the development of another.

The recently published findings demonstrate that very early human cells can be created and then successfully integrated into mice, owing to their undifferentiated, or nave], state, wherein they can develop into any type of cell in the body, including other stem cells. Additionally, the researchers lay out a protocol for significantly increasing the efficiency (or competence) with which these cells can integrate. Improving our ability to create and study these cell types could be used in the future to transfer cells if not organs from one animal to another, humans included.

Hannas lab broke ground in 2013 when they were the first to inject human stem cells into mice and show that they can successfully integrate into the latters developing embryos. Eight years after this study was first published, Hanna and his team felt that they could go one step further by attempting to produce an even earlier, fully naive, form of stem cells for use in similar procedures. As they were mulling over the idea, Hanna knew that this might be nearly, if not altogether, impossible to achieve. Our experience with producing similar cells in mice has taught us to expect challenging obstacles along the way, says Hanna.

These cells normally suffer from genetic, as well as epigenetic, instability, and in the end they dont differentiate too well, which is key to proper embryonic development and a prerequisite for their integration into another animals embryo. In fact, only about 1-3 percent of cells that have been transferred between species actually manage to integrate and contribute to development.

To boost these numbers, the researchers in the new study inhibited two additional signaling pathways to produce naive human stem cells having a stable genome, relatively few gene regulation glitches, and most importantly, they differentiate perfectly. The researchers also mutated an important gene that contributes to genome stability, which resulted in not only competent, but also competitive, stem cells that can integrate well without causing damage to the host. We found a way to make human stem cells more competent, and competitive, increasing the chances for a successful transfer by about fivefold compared to what we were able to do in the past, concludes Hanna.

While the previous study showed that human naive stem cells can differentiate into primordial germ cells the progenitors of egg or sperm cells the fully naive stem cells produced in the present study can also differentiate into extraembryonic tissues, the placenta and yolk sac cells that sustain the developing embryo. Such cells could be used, for example, as the source for developing synthetic embryos without the need for donor eggs. Reaching this state with mouse stem cells is particularly difficult to accomplish, Hanna explains, noting that human cells are apparently different.

This is perhaps the most surprising finding that the researchers made highlighting the differences between the behavior of human and mouse stem cells, and between the different states of naive cells. These differences expose the work that still needs to be done in making the dream of developing made-to-order organs a real-world actuality.

According to Hanna, understanding these differences will be pivotal for overcoming a myriad of issues still facing the field of stem cell research and application: If in the future we should wish to grow a pancreas in pigs for human transplantation, for example, we will have to take into account these massive evolutionary differences between species, beginning with mice and humans. For now, it would seem that Hanna and his team have taken a constructive leap in that direction.

Glaser, MD Award for Innovative Research in Retina. The award recognizes an early-career investigator who has made a novel discovery that impacted the understanding and/or treatment of a retinal disease or condition.

Renner leads the Human Organoid Platform at the Institute of Molecular and Clinical Ophthalmology Basel (IOB), Basel, Switzerland. Together with her team she is optimizing the culture of retinal organoids as a model for the human retina and retinal disease. She is particularly interested in enabling the mass production of retinal organoids and other cell types of the human eye to facilitate high-content compound screening and testing of novel gene therapy tools.

Renner is also the recipient of the Swiss OphthAWARD Best Experimental Work (2021), the PRO RETINA award of RETINA Suisse and PRO RETINA Germany (2021) and won the 1st place of the VSY Biotechnology Ophthalmology Star Award (2021).

Recipients of the Bert M. Glaser, MD Award must demonstrate their work has led to the development of a new drug or technique that will revolutionize retina care. Renner and her team have developed a method to generate thousands of artificial human retinas (retinal organoids) from human pluripotent stem cells. "We compared retinal organoids to post-mortem human retina and analyzed their cell type-diversity, morphology, light-responses and gene-expression," says Renner. "Importantly, we analyzed which cell types express known disease genes in both human retina and retinal organoids which validates retinal organoids as a promising new model to study retinal disease."

Funded by the Glaser family through the ARVO Foundation, the $10,000 award is named in memory of Bert M. Glaser, MD, a long-time retina researcher, surgeon, innovator and teacher whose life was cut short in 2017 after a brief illness.Read more about Bert M. Glaser.

Jake’s mice: Searching for answers to the puzzle of autism – Associated Press

ST. LOUIS (AP) Jake Litvag leaned in for a closer look as a lab mouse scurried around an enclosure, stopping to sniff a large block.

Hi, Jakob 1. Im Jake, the 16-year-old said, naming the little furry creature engineered to have the same genetic abnormality he has.

That mouse and its lab-grown relatives are the first in the world to mirror the missing gene that causes Jakes autism. Scientists at Washington University in St. Louis bred the mice, and grew stem cells derived from Jakes blood, to study and find ways to treat his rare disorder and look for answers to the larger puzzle of autism.

Jakes family raised money for the early research, which scientists then parlayed into a $4 million grant from the National Institutes of Health to delve more deeply into Jakes gene, one of more than 100 implicated in autism. They hope to find points of convergence that could someday help people with all forms of the neurodevelopmental condition affecting one in 44 U.S. children.

Jake knows he inspired their work. And thats helped him see autism as something to be proud of rather than something that makes him different from other kids. His parents, Joe and Lisa Litvag, figured meeting the scientists and the mice would show him firsthand what he had brought into being.

Oh wow. Cool! Jake said as he watched a mouse climb down a pole while others scampered in a bin.

Walking out of the lab, tears welled up in Lisa Litvags eyes as she thought about the language within her sons cells helping other kids.

Were deeply proud and humbled to be part of this, said Joe Litvag. What do we live this life for? Its ultimately to try to, in one way, shape or form, be of service to others.

JAKES GIFT

The Litvags realized early on that Jake wasnt reaching childhood milestones. He couldnt walk without assistance until he was 4. He struggled to string sentences together in first grade.

At first, no one could pin down why. Jake had a mix of different traits. He was hyperactive and impulsive but also social, warm and funny. It took until he was 5 to get a firm diagnosis of autism.

Around that time, the Litvags heard that child psychiatrist Dr. John Constantino, an expert on the genetic underpinnings of autism, was giving a talk at the Saint Louis Science Center. They decided to go in the hopes of meeting him. They did, and he began seeing Jake as a patient.

About five years later, Constantino proposed genetic testing. It revealed the missing copy of the MYT1L gene believed to cause one out of every 10,000 to 50,000 autism cases. Having an extra copy can cause schizophrenia.

The finding brought the family peace. Theyd heard lots of people say autism was mostly caused by external factors, like birth trauma. For a long time, Lisa Litvag said, I thought it was something that I did.

Actually, a large multinational study suggests that up to 80% of the risk for autism can be traced to inherited genes.

One of the big things it did for us as a family is it made us realize that its nothing that we did wrong, Joe Litvag said. Its just that people are born all the time with genetic differences.

The couple, whose younger son Jordan doesnt have the condition, talked openly with Jake about his autism and tried to bolster his self-esteem when he worried about being seen as different. They sent him to a small private school that tailors its curriculum to each childs learning abilities. And they encouraged his social tendencies, cheering him on when he and some classmates formed a band, the Snakes.

We never wanted him to feel there was shame around his diagnosis, Lisa Litvag said. We continued to kind of reinforce that this is a superpower, you are special, you are awesome and because you have autism, there are gifts you have to give other people.

GIFTS BLOSSOM

When Constantino suggested studying the little-understood MYT1L gene, the Litvags enthusiastically agreed to help. Constantino who is on the local board of a group theyve long been active in called Autism Speaks asked if theyd be interested in raising money for early research.

Joe Litvag, an executive in the live music industry, and Lisa Litvag, a partner in a marketing firm, reached out to family and friends and raised the $70,000 needed in about six months.

With half the money, researcher Kristen Kroll and her team reprogrammed cells from Jakes blood into induced pluripotent stem cells, which can be prodded into becoming various cell types. With the other half, scientist Joseph Dougherty and his team followed the blueprint of Jakes genome and induced his mutation in mice using the gene-editing tool CRISPR.

Like the people theyre meant to model, mice with the mutation tended to be more hyperactive than siblings without it, running around their cages much more. They were nonetheless generally heavier, especially the first generation of mice. They had slightly smaller brains and a little less of the white matter that speeds communication between different brain regions.

Since starting the research about three years ago, scientists have bred around 100 mice with Jakes mutation and are now using the great-great grandchildren of the first one they engineered. They recently published about the mice in the journal Neuron.

While scientists cant go back and see how Jakes brain developed, Dougherty said, mice allow them to watch the mutation play out through generations.

A GIFT IN RETURN

Dougherty and his colleagues hope what they learn about how MYT1L functions ultimately leads to medicines or gene therapies that improve or even correct the problems the mutation causes.

They are sharing their findings with scientists studying other autism-causing genes or trying to figure out how various genes work together to cause the condition. According to the Simons Foundation Autism Research Initiative, more than 100 genes have strong evidence linking them to autism and a growing list contains several hundred more genes thought to be linked to the condition.

In cases where autism is caused by a single gene, Dougherty said that gene probably does many things to brain development. A key to understanding autism overall is to find one or two things shared across different forms of autism - which could then be targets for treatment. Though not everyone with autism wants treatment, Dougherty said it could help those who do.

Since the research began, Dougherty has been writing notes to the Litvags explaining the latest discoveries. But as a lab scientist, hes mostly removed from the people sparking the research and first met the family when they were invited by the school to visit in December.

After meeting the mice, they stopped into another lab, where Jake peered through a microscope at his blue-stained stem cells.

Thats me! Thats cool stuff. I never saw anything like that in my life, he said, stepping back to lean into his dad, who pulled him close.

Dougherty used the visit as an opportunity to share some news, a gift of sorts that he wanted to tell the family in person.

The missing gene doesnt seem to shorten life. The mice live 2 to 3 years, the same as their siblings.

So, a normal life span? Joe Litvag asked hopefully.

Yes, Dougherty answered. As far as we can tell, identical. I know thats a big relief, too.

Joe Litvag turned to his son. So Jake, maybe you will live to be 100.

I will be 112! Jake replied with a grin.
